CONTACTING OUR INSURANCE DEPARTMENT:
Please contact the Insurance Verification Department prior to your surgery/procedure to discuss insurance
benefits or to ask any financial questions. You may reach us by phone at 863-683-2428 or by e-mail at
verifiers@lsdc.net. In most cases we should be able to estimate the cost of your surgery/procedure beforehand.
However, this will depend on when your surgery/procedure is scheduled by your physician’s office.
We will contact you either by phone or letter to make you aware of your patient responsibility. Your
responsibility includes Co-payments, Deductibles, Co-Insurance and Out of Pocket Amount as determined by
your insurance carrier. This amount will need to be paid on or before your surgery/procedure visit.
 CHARGES:
Charges for Lakeland Surgical & Diagnostic Center and Physician are separate. The facility charges include
use of the operating room, nursing staff, equipment and most supplies. Your surgeon, anesthesiologist,
radiologist and pathologist (if applicable) will bill you separately for their services.
PAYMENTS:
Uninsured and/or self-pay patients will be required to pay for services on or before the admission date.
Patients with verified insurance will be required to pay ONLY their estimated portion on or before the day
of their surgery/procedure.
For your convenience pre-payment can be made prior to your visit by clicking on the "Pay Online" icon at the top
of this page, or by contacting the Insurance Verification department at 863-683-2428.
Cash, Check, MasterCard, Visa, American Express and Discover are accepted.
We will bill your insurance company as a courtesy; however, the balance due is the patient’s responsibility.
Medicaid patients must have a current valid medical card with them at the time of admission.
The Center may be willing to work with patients on an individual basis in regards to financial matters.
