Tricare as Primary: Lakeland Surgical is not currently under contract with Tricare. However LSDC will offer Tricare patients the current Medicare rate upfront and provide a claim form in order for the patient to bill Tricare directly. The claim form will be sent to the patient after their claim has been entered in the billing system.
Please Note: Lakeland Surgical & Diagnostic Center will accept all non-contracted Insurances as long as the Patient's Policy has Out-of-Network benefits or the patient is willing to be responsible for full billed charges.
If your Carrier is not listed, please call our Insurance Verification Department with any questions regarding patient benefits. LSDC will review cases individually to determine if the case can be performed in an ASC setting.
