contract with Tricare. However LSDC will offer Tricare
patients the current Medicare rate upfront and provide
a claim form in order for the patient to bill Tricare
directly. The claim form will be sent to the patient
after their claim has been entered in the billing system.

Lakeland Surgical & Diagnostic Center
will accept all non-contracted Insurances as long
as the Patient's Policy has Out-of-Network benefits
or the patient is willing to be responsible for full
billed charges.
