Patient Rights
PATIENT RIGHTS
The Center will provide each patient with verbal and written information regarding Patient Rights in advance of the date of the procedure to include notice of rights, physician financial interests or ownership in the ASC, advance directives, submission and investigation of grievances, exercise of rights and respect for property and person, privacy and safety, confidentiality of clinical records.
  • The patient will receive a copy of the Florida Patient's Bill of Rights at the time of admission.
  • Patient Rights are posted in the patient visitor lobby for reference in Spanish and English.
  • Physician financial interests or ownership in the ASC facility is included in the patient's packet of preprinted information.
  • In the event that a patient requests to change their physician or their anesthesiologist, the attending physician and /or anesthesiologist would be notified. In the event that LSDC is unable to make such changes for that date or service, the patient would be given the option to reschedule to accommodate their request.
  • During admission the patient will be given the opportunity to submit a completed Advance Directive.
  • If the Advance Directive contains a do not resuscitate provision, the patient will be notified that the provision will be suspended during his/her surgical procedure and recovery in the facility.
  • If Advance Directive documents are placed on the chart and the patient is subsequently transferred to a higher level of care, the presence of Advance Directives must be clearly communicated during hand off.
  • Appropriate documentation will be noted on the patient consent and electronically.
Grievances
  • The Center has established grievance procedures for documenting the existence, submission, investigation, and disposition of a patient's written or verbal grievance. Please direct comments, suggestions, or complaints to the Privacy Officer @ 863-683-2428 or by e-mail to info@lsdc.net
Exercise of rights and respect for property and person
  • The patient has the right to -
    1. exercise rights without subjection to discrimination or reprisal.
    2. voice grievances regarding treatment or care.
    3. be informed about treatment or procedure and risks and benefits as conducted when obtaining patient clinical consent.
  • Adjudged incompetent patient right's shall be exercised by the court appointed person. Proper documentation shall be obtained and filed in the patient's permanent medical record.
  • A patient has the right to designate any legal representative in accordance with State law to exercise his/her rights to the extent allowed by State law i.e. through Healthcare Surrogate form and advance directive information packet provided to the patient.
Privacy and safety
  • The patient will be provided additional privacy rights as outlined in the HIPAA Notice of Patient Privacy Practices which is provided to the patient upon admission and posted in the Center lobby.
  • Personnel are trained on privacy and safety standards in order to provide the patient with care in a safe and private setting free from abuse or harassment.
Confidentiality of clinical records
  • Medical records are protected in accordance with privacy and security standards as specified by 45 CFR parts 160 and 164.
